Consent Agreement 

You are initiating an inquiry with Initial Consulting Group, LLC, operating under the name My Debt Relief Center, along with their affiliated entities (collectively referred to as the "Company"), for various financial services. To comply with legal requirements and offer you services, the Company must obtain your consent to provide certain communications, notices, disclosures, and information (referred to as "Communications") electronically. Additionally, your electronic agreement with the Company includes acceptance of the associated Terms of Use.

  • Adobe Acrobat Reader version 6 or higher.

  • Printing capability.

  • A long-term storage device, such as your computer's hard drive, to save copies of the Communications.

  1. Requesting Paper Records

You have the option to request paper copies of any Communications at no cost to you. To do so, contact the Company at 1-888-994-0807 or submit a written request to one of the addresses listed below, including specific details about your request:

  1. Withdrawing Consent

You cannot withdraw your consent during the inquiry process, as it may be required for the Company to respond to your inquiry. However, after submitting your inquiry, you have the option to withdraw your consent to conduct electronic business related to the inquiry with the Company at no cost. If you choose to do so, you can receive future Communications and Disclosures in paper form via U.S. mail at no charge by contacting the numbers provided above. Withdrawing your consent will not affect the legal validity of previous electronic Communications and Disclosures provided to you.

It is important to keep your contact information up to date with the Company. You can update your information by logging into the Company's website or by sending an electronic mail update to info@initialconsultinggroup.com.

  1. Legal Implications

By clicking the provided link, you are consenting to the terms outlined in this document. You acknowledge that you have read and understood the information about electronic signatures, records, disclosures, and conducting business electronically. You consent to the use of electronic signatures, the provision of all Disclosures in electronic form, and conducting business with the Company electronically. You also recognize your right to request paper copies of electronic records and Disclosures at no charge. If you choose not to proceed, it indicates that you do not wish to use electronic signatures or conduct this transaction electronically. Additionally, you acknowledge that your consent to electronic Disclosures is necessary to receive services from the Company over the internet.
